Leave of Absence

During your employment with the Shoreline School District, you may need to take time off for your own health issues, or to care for a family member.

There are a number of district, local and federal policies and provisions that may provide options for you when you need to take a leave of absence.

Process & Forms

Please contact Human Resources, Marie McCluskey, marie.mccluskey@ssd412.org, as soon as you become aware of the need to be off from work for medical reasons.  This includes personal illness or injury, maternity leave, paternity leave, or to care for a family member.

You must complete in advance a leave request form,  and provide medical documentation  for the time period that you will need to be off from work.

Once we receive the forms above, we can help you determine that best option for your leave.

If you are requesting Shared Leave, you must complete this form, and provide doctor's documentation (if not previously provided).  Please send this request to marie.mccluskey@ssd412.org  

Ready to return for your Leave of Absence?

Health Related Absences

Once you are ready to return from your health related leave of absence, please provide documentation from your healthcare provider, stating that you are ready to return to full duty. 

If your doctor recommends any restrictions or "light duty" for your return, we must have these restrictions documented on our Return to Work Form.

Restrictions may include:  using crutches, limiting your lifting, working reduced hours, or working fewer days per week.  

Once we have this form, we will review your restrictions with your Administrator, and we will meet with you, to discuss how to reasonably accommodate your restrictions.
