Placement Change and Exit of Services Form - This is for students who want to change their service location if qualified and or exit the program on an annual basis. (pdf version- print, sign and return to the Highly Capable  Department)

Referral Form- Referring a student for highly capable services within Shoreline Public Schools. Any staff member, student or community member may refer a Shoreline student in Grades 2, 4 & 6-8 for hi-cap evaluation by completing this online form. (Google form)
